About Delta Maternal Care Coordination

The Delta Maternal Care Coordination (Delta MCC) grant is a new program that aims to improve and increase access to pre- and postpartum care for pregnant women and new mothers in rural West Tennessee.  Delta MCC utilizes maternal care navigators to help assist clients in connecting to resources across eighteen counties.

Program goals include:

1. Build the capacity and infrastructure of the network and its members for increasing access to coordinated perinatal services in rural West Tennessee.

2. Identify propriety resources and strategies to address critical barriers or gaps in the regional maternal and perinatal care system.

3. Identify evidence based, promising practices and/or value- based model(s) that improve coordinated maternal care and perinatal services.

4. Develop and implement sustainable strategies of care coordination into Network, member policies/procedures and infrastructure.

Eligibility:

Any expectant or new parent residing in any of the following West Tennessee counties: Benton, Carroll, Chester, Crockett, Decatur, Dyer, Gibson, Hardeman, Hardin, Haywood, Henderson, Henry, Lake, Lauderdale, McNairy, Obion, Tipton, Weakley
