Several programs are available to all West Tennesseans who need help managing their chronic diseases.
HCCHC is a federally-qualified health center partnered with GrowWell to provide coordination of health services and education about managing chronic conditions, specifically diabetes. These services usually take place while visiting the clinic to see your healthcare provider. A wide-range of assistance is available at no additional cost including:
Henry County provides similar health education and disease management for chronic conditions, specifically obesity-related conditions such as heart disease, hypertension, and diabetes. Education and services are provided by an LPN Community Health Specialist in conjunction with your healthcare provider and healthcare team.
The Chronic Disease Management program at Henry County also provides quarterly health screenings to determine disease states and improvements. Testing is free of charge and includes such things as fasting blood sugar, total cholesterol, LDL, HDL, triglycerides, and hemoglobin A1c (if indicated). A personalized education plan is formulated for each person based on identified needs.
Group education sessions are also available as need arises including things such as cooking, food budgeting, meal planning, quitting smoking and health eating.
These services are available for all West Tennesseans at these locations.
